First Communion, Confirmation, OCIC & OCIA

1st Communion

  • Current Baptismal Certificate

Confirmation Candidate

  • Current Baptismal Certificate to include 1st Communion Certificate
  • Sponsr

OCIC Youth

  • Birth Certificate (if needing baptismal)
  • Newly Issued Baptismal Certificate* (if needing 1st Communion and Confirmation)
  • Newly Issued Baptismal Certificate and 1st Communion Certificate* (if needing Confirmation only)
  • Saint Name
  • Sponsor

Dress Code for Day of Confirmation

  • Boys—White shirt, black slacks and tie
  • Girls—White dress is preferred
  • Length cannot be shorter than 1 inch above the knee
  • Shoulders must be covered—no sleeveless or spaghetti straps, no bare shoulders
  • No gloves

Requirements to be a Sponsor

  • Must be baptized Roman Catholic
  • If married, must be married in the Roman Catholic Church
  • If single, not living with someone or married outside the Catholic Church
  • Must be at least sixteen (16) years of age
  • Confirmed in the Catholic Church
  • Must attend Mass regularly and must be living in full communion with the Catholic Church
  • Must provide a newly issued Baptismal Certificate to include 1st Communion, Confirmation, and marriage if applicable
  • A marriage license is not acceptable as proof of sacraments

*Current Baptismal Certificates

  • A newly issued Baptismal Certificate will include your 1st Communion, Confirmation, and Marriage (if applicable) on the back of the certificate
  • The original decorative certificate you received at the time of the sacrament is not acceptable. It must be current
  • A current Baptismal Certificate is obtained from the Church where you were baptized. They should have all your sacraments recorded with your baptismal record

Below is an image of a sample Baptismal Certificate. Both front and back must be filled out by the church, signed by the priest, and stamped with their seal.

Some Churches take about a month to search their records for your information, so please contact them as soon as possible.
